We offer a variety of ministry opportunities for both Middle School and High School students in grades 6-12. We would love to have you join us!

2022-2023 Youth Ministry Release Forms

All forms are digital. Please enter Kelli Tull-Waldrop email KTullWaldrop@HolyFamilyFW.org in the space designated for Youth Minister and it will be sent to her.

Form A

Form A is good for the whole year from June 1, 2022 to May 31, 2023. Please fill it out completely as it will stay on file in the Youth Ministry office for the year and be used for all events involving youth.

Form B

Form B is required for EACH event and we must have one per person. This is similar to the "permission slip" in school, granting permission to attend the event and includes the Code of Conduct. This Form goes with Form A to every event.

Form D

Form D is for ADULTS who are 18 or older AND out of High School. It also is good for the whole year from June 1, 2022 to May 31, 2023. Please fill it out completely as it will stay on file in the Youth Ministry office for the year and be used for all events you attend.

Driver Form

Driver Form The Driver Form must be completed for anyone driving to and from a youth ministry sponsored event. This form stays on file in the Youth Ministry Office.

Confirmation

The Sacrament of Confirmation is a gift freely given and freely received. In the Diocese of Fort Worth, we invite young people in the 10th grade and older to enter into the preparation process to receive the Sacrament of Confirmation. We invite you to join us as we journey together towards affirming your gifts of the Holy Spirit and confirming your relationship with Christ and His Holy Catholic Church.

 

Eligibility:

  • Students must be at least 15 years old and a Sophomore in High School in order to participate.
  • Students must be registered members of the parish community or receive approval from their pastor in writing.
  • Students need to be attending Religious Education classes through the parish Faith Formation program
  • Students will need to select a Sponsor to walk the faith journey with them (more information will be given on this at the Orientation)

Parents:

Parents play an important role in the spiritual journey of their children. Their role is no less important as these children become young men and women. It is important that parents continue to set a positive example and mentor their students through this part of their faith journey. Read more 

Sponsors:

Sponsors journey with their Candidates towards the celebration of the Rite of Confirmation serving as a mentor and role model for Christian living. Along with the Candidate's parents, Sponsors guide, encourage, challenge and support their Candidate as they grow in faith. Read more
